Background: Clinical genomic implementation studies pose challenges for informed consent. Consent forms often include complex language and concepts, which can be a barrier to diverse enrollment, and these studies often blur traditional research-clinical boundaries. There is a move toward self-directed, web-based research enrollment, but more evidence is needed about how these enrollment approaches work in practice. In this study, we developed and evaluated a literacy-focused, web-based consent approach to support enrollment of diverse participants in an ongoing clinical genomic implementation study. (...) Methods: As part of the Cancer Health Assessments Reaching Many (CHARM) study, we developed a web-based consent approach that featured plain language, multimedia, and separate descriptions of clinical care and research activities. CHARM offered clinical exome sequencing to individuals at high risk of hereditary cancer. We interviewed CHARM participants about their reactions to the consent approach. We audio recorded, transcribed, and coded interviews using a deductively and inductively derived codebook. We reviewed coded excerpts as a team to identify overarching themes. Results: We conducted 32 interviews, including 12 (38%) in Spanish. Most (69%) enrolled without assistance from study staff, usually on a mobile phone. Those who completed enrollment in one day spent an average of 12 minutes on the consent portion. Interviewees found the information simple to read but comprehensive, were neutral to positive about the multimedia support, and identified increased access to testing in the study as the key difference from clinical care. Conclusions: This study showed that interviewees found our literacy-focused, web-based consent approach acceptable; did not distinguish the consent materials from other online study processes; and valued getting access to testing in the study. Overall, conducting empirical bioethics research in an ongoing clinical trial was useful to demonstrate the acceptability of our novel consent approach but posed practical challenges. (shrink)

We propose a framework for analyzing the development, operation and failure to survive of all things, living, non-living or organized groupings. This framework is a sequence of developments that improve survival capability. Framework processes range from origination of any entity/system, to the development of increased survival capability and development of life-forms and organizations that use intelligence. This work deals with a series of developmental changes that arise from the uncovering of emergent properties. The framework is intended to be general, but (...) we see a potential to apply it to scientific topics such as the exploration of the origin of life or the search for life beyond Earth, and to understand some biological issues in evolution and symbiosis, and also to apply to social systems that do not seem to be operating well, to determine their problems and correct them. Context: Second-order cybernetics and its implications have been understood within the cybernetics community for some time. These implications are important for understanding the structure of scientific endeavor, and for researchers in other fields to see the reflexive nature of scientific research. This article is about the role of context in the creation and exploration of our experience. Problem: The purpose of this article is to point out the fundamental nature of the circularity in cybernetics and in scientific work in general. (...) I give a point of view on the nature of objective knowledge by placing it in the context of reflexivity and eigenform. Method: The approach to the topic is based on logical analysis of the nature of circularity. Mathematics and cybernetics are both fundamentally concerned with the structure of distinctions, but there can be no definition of a distinction without circularity, since such a definition would itself be a distinction. The article proceeds by explicating the structure of reflexive domains D where the transformations of the domain are in one-to-one correspondence with the domain itself. Results: I show that every element of a reflexive domain has a fixed point. This means that eigenforms arise naturally in reflexive domains. Furthermore, a reflexive domain is itself an eigenform (at a higher level. This supports the context of a second-order science that would study domains of science as part of a larger cybernetic landscape. Implications: The value of the article is in its concise reformulation of the scientific endeavor as a search for eigenforms in reflexive domains. This new view of science is promising in that it includes the former worlds of apparent objectivity and it embraces those newer worlds of science where the theories and theorists become active participants in the ongoing process of creating knowledge. Constructivist content: I argue that the perspective of reflexive domains constitutes a new way to think about the practice of science, with observers deeply imbedded, and objectivity understood as the mutual search for eigenforms. Upshot: Eigenform is a precondition for distinction and distinction is a precondition for eigenform. While my target article discussed eigenform and reflexivity, it could just as well have discussed distinctions and the emergence of distinctions. This theme was implicit in many of the comments. We make this circularity explicit for the sake of a deeper understanding.

Context: The question of how to understand the epistemology of set theory has been a longstanding problem in the foundations of mathematics since Cantor formulated the theory in the 19th century, and particularly since Bertrand Russell articulated his paradox in the early twentieth century. The theory of types pioneered by Russell and Whitehead was simplified by mathematicians to a single distinction between sets and classes. The question of the meaning of this distinction and its necessity still remains open. Problem: I (...) am concerned with the meaning of the set/class distinction and I wish to show that it arises naturally due to the nature of the sort of distinctions that sets create. Method: The method of the paper is to discuss first the Russell paradox and the arguments of Cantor that preceded it. Then we point out that the Russell set of all sets that are not members of themselves can be replaced by the Russell operator R, which is applied to a set S to form R(S), the set of all sets in S that are not members of themselves. Results: The key point about R(S) is that it is well-defined in terms of S, and R(S) cannot be a member of S. Thus any set, even the simplest one, is incomplete. This provides the solution to the problem that I have posed. It shows that the distinction between sets and classes is natural and necessary. Implications: While we have shown that the distinction between sets and classes is natural and necessary, this can only be the beginning from the point of view of epistemology. It is we who will create further distinctions. And it is up to us to maintain these distinctions, or to allow them to coalesce. Constructivist content: I argue in favor of a constructivist perspective for set theory, mathematics, and the way these structures fit into our natural language and constructed speech and worlds. That is the point of this paper. It is only in the reach for absolutes, ignoring the fact that we are the authors of these structures, that the paradoxes arise. (shrink)

The twenty‐six‐year‐old patient requested a prophylactic bilateral mastectomy with reconstruction because of an extensive family history of cancer. She reported that she had developed melanoma at twenty‐five; that her mother, sister, aunts, and a cousin all had breast cancer; that a cousin had ovarian cancer at nineteen; and that a brother was treated for esophageal cancer at fifteen. The treating team was skeptical about this history, and they could find no documentation of the patient's reported melanoma. The surgeon wrote the (...) patient's primary care physician, explaining that he had seen the patient and planned to proceed with the bilateral mastectomy and reconstruction. The primary care physician responded that he was unable to substantiate several of the patient's claims; some of his colleagues believed the family history to be fabricated. Trying to make sense of the discrepancies, the genetic counselor called a counselor colleague who had met with this patient. The colleague suggested that the in‐house genetic counselor “google” the patient. The search revealed two Facebook pages linked to the patient. In one, apparently a personal profile, the patient stated that in addition to battling stage four melanoma, she had recently been diagnosed with breast cancer. She provided a link to a site where she solicited donations to attend a summit for young cancer patients. The other Facebook page featured numerous pictures of her with a bald head, as though she has been through chemotherapy. The genetic counselor showed the Facebook pages to the surgeon, who then decided not to operate. Should health care professionals “google” their patients? (shrink)

Les différents travaux de Frédéric Rasera, explorant les conditions sociales individuelles de la recomposition vocationnelle chez un ex-apprenti footballeur, font office d’exception. Face à ce constat, cet article entend contribuer à la réflexion portant sur ces jeunes sportifs. À travers l’étude des trajectoires d’une vingtaine d’ex-apprentis footballeurs, nous verrons qu’il est possible de trouver des similarités biographiques caractérisant leur processus de recomposition vocationnelle. This paper presents a speculative model of the cognitive mechanisms underlying the transition from episodic to mimetic (or memetic) culture with the arrival of Homo erectus, which Donald [1991] claims paved the way for the unique features of human culture. The model draws on Kauffman's [1993] theory of how an information-evolving system emerges through the formation of an autocatalytic network. Though originally formulated to explain the origin of life, this theory also provides a plausible account of how discrete episodic (...) memories become woven into an internal model of the world, or worldview, that both structures, and is structured by, self-triggered streams of thought. Social interaction plays a role in (and may be critical to) this process. Implications for cognitive development are explored. (shrink)

Though early ideas of evolution saw it as progressive, most modern theories see it as a random walk. The theories of Richard Dawkins, Stephen Jay Gould, Edward O. Wilson, Stuart Kauffman, Steven Rose, and Robert Wesson are surveyed, showing their agreement on the fact of evolution but not on the mechanism. Evolution is an incomplete theory. Any theology should therefore be based only on its broadest features. Generally, evolution is the development of complex forms from simple ancestors. Within a (...) Christian context, it can be seen as a journey toward the unification of all things in Christ, the ultimate complexity. (shrink)

According to James M. Kauffman, too much of what is said today about educational reform is nonsense that shortchanges students, parents, and taxpayers. This deforms education rather than reforming it. The primary objective of this book is to help teachers, teacher educators, policy makers, and parents think more critically about current rhetoric about education. Reason and science in the enlightenment tradition are more helpful in reforming and improving education than political agendas. Reform should focus on instruction. Education must address (...) the full range of learners, from those who are mentally retarded to those who are intellectually gifted. Special education, multicultural education, and standardized testing are among the controversial issues explored. Extremes of both left and right ideologies are rejected in favor of careful thinking and sound judgment. (shrink)

Ethical lapses associated with the first facial transplant included breaches of confidentiality, bending of research rules, and film deals. However, discussions of the risk-benefit ratio for face transplantation are often deficient in that they ignore the needs, experience, and decision-making capability of potential recipients.

Attempts to ‘naturalize’ phenomenology challenge both traditional phenomenology and traditional approaches to cognitive science. They challenge Edmund Husserl’s rejection of naturalism and his attempt to establish phenomenology as a foundational transcendental discipline, and they challenge efforts to explain cognition through mainstream science. While appearing to be a retreat from the bold claims made for phenomenology, it is really its triumph. Naturalized phenomenology is spearheading a successful challenge to the heritage of Cartesian dualism. This converges with the reaction against Cartesian thought (...) within science itself. Descartes divided the universe between res cogitans, thinking substances, and res extensa, the mechanical world. The latter won with Newton and we have, in most of objective science since, literally lost our mind, hence our humanity. Despite Darwin, biologists remain children of Newton, and dream of a grand theory that is epistemologically complete and would allow lawful entailment of the evolution of the biosphere. This dream is no longer tenable. We now have to recognize that science and scientists are within and part of the world we are striving to comprehend, as proponents of endophysics have argued, and that physics, biology and mathematics have to be reconceived accordingly. Interpreting quantum mechanics from this perspective is shown to both illuminate conscious experience and reveal new paths for its further development. In biology we must now justify the use of the word “function”. As we shall see, we cannot prestate the ever new biological functions that arise and constitute the very phase space of evolution. Hence, we cannot mathematize the detailed becoming of the biosphere, nor write differential equations for functional variables we do not know ahead of time, nor integrate those equations, so no laws “entail” evolution. The dream of a grand theory fails. In place of entailing laws, a post-entailing law explanatory framework is proposed in which Actuals arise in evolution that constitute new boundary conditions that are enabling constraints that create new, typically unprestatable, Adjacent Possible opportunities for further evolution, in which new Actuals arise, in a persistent becoming. Evolution flows into a typically unprestatable succession of Adjacent Possibles. Given the concept of function, the concept of functional closure of an organism making a living in its world, becomes central. Implications for patterns in evolution include historical reconstruction, and statistical laws such as the distribution of extinction events, or species per genus, and the use of formal cause, not efficient cause, laws. Biological evolution is a complex blend of ever changing structural stability, variability and emergence of new phe- notypes, niches, ecosystems. We wish to argue that the evo- lution of life marks the end of a physics world view of law entailed dynamics. Our considerations depend upon dis- cussing the variability of the very ”contexts of life”: the in- teractions between organisms, biological niches and ecosys- tems. These are ever changing, intrinsically indeterminate and even unprestatable: we do not know ahead of (...) time the ”niches” which constitute the boundary conditions on selec- tion. More generally, by the mathematical unprestatability of the ”phase space”, no laws of mo- tion can be formulated for evolution. We call this radical emergence, from life to life. The purpose of this paper is the integration of variation and diversity in a sound concep- tual frame and situate unpredictability at a novel theoretical level, that of the very phase space. Our argument will be carried on in close comparisons with physics and the mathematical constructions of phase spaces in that discipline. The role of symmetries as invariant preserving transformations will allow us to under- stand the nature of physical phase spaces and to stress the differences required for a sound biological theoretizing. In this frame, we discuss the novel notion of ”enablement”. Life lives in a web of enablement and radical emergence. This will restrict causal analyses to differential cases. Mutations or other causal differ- ences will allow us to stress that ”non conservation princi- ples” are at the core of evolution, in contrast to physical dynamics, largely based on conservation principles as sym- metries. Critical transitions, the main locus of symmetry changes in physics, will be discussed, and lead to ”extended criticality” as a conceptual frame for a better understanding of the living state of matter. The rhetoric of the social model of disability is presented, and its basic claims are critiqued. Proponents of the social model use the distinction between impairment and disability to reduce disabilities to a single social dimension—social oppression. They downplay the role of biological and mental conditions in the lives of disabled people. Consequences of denying biological and mental realities involving disabilities are discussed. People will benefit most by recognizing both the biological and the social dimensions of disabilities.

Ultimately we will only understand biological agency when we have developed a theory of the organization of biological processes, and science is still a long way from attaining that goal. It may be possible nonetheless to develop a list of necessary conditions for the emergence of minimal biological agency. The authors offer a model of molecular autonomous agents which meets the five minimal physical conditions that are necessary (and, we believe, conjointly sufficient) for applying agential language in biology: autocatalytic reproduction; (...) work cycles; boundaries for reproducing individuals; self-propagating work and constraint construction; and choice and action that have evolved to respond to food or poison. When combined with the arguments from preadaptation and multiple realizability, the existence of these agents is sufficient to establish ontological emergence as against what one might call Weinbergian reductionism. Minimal biological agents are emphatically not conscious agents, and accepting their existence does not commit one to any robust theory of human agency. Nor is there anything mystical, dualistic, or non-empirical about the emergence of agency in the biosphere. Hence the emergence of molecular autonomous agents, and indeed ontological emergence in general, is not a negation of or limitation on careful biological study but simply one of its implications. The literatures on bounded and ecological rationality are built on adaptationism—and its associated modular, cognitivist and computational paradigm—that does not address or explain the evolutionary origins of rationality. We argue that the adaptive mechanisms of evolution are not sufficient for explaining human rationality, and we posit that human rationality presents exaptive origins, where exaptations are traits evolved for other functions or no function at all, and later co-opted for new uses. We propose an embodied reconceptualization of rationality—embodied rationality—based on the (...) reuse of the perception-action system, where many neural processes involved in the control of the sensory-motor system, salient in ancestral environments have been later co-opted to create—by tinkering—high-level reasoning processes, employed in civilized niches. Older adults in the United States have been the age group hardest hit by the Covid pandemic. They have suffered a disproportionate number of deaths; Covid patients eighty years or older on ventilators had fatality rates higher than 90 percent. How could we have better protected older adults? Both the popular press and government entities blamed nursing homes, labeling them “snake pits” and imposing harsh fines and arduous new regulations. We argue that this approach is unlikely to improve protections for (...) older adults. Rather than focusing exclusively on acute and critical resources, including ventilators, a plan that respected the best interests of older adults would have also supported nursing homes, a critical part of the health care system. Better access to protective equipment for staff members, early testing of staff members and patients, and enhanced means of communication with families were what was needed. These preventive measures would have offered greater benefit to the oldest members of our population than the exclusive focus on acute care. (shrink)

The Institute of Medicine’s 2005 publication, Dietary Supplements: A Framework for Evaluating Safety, is authoritative and thorough, and thus representative of other reports by the Institute of Medicine. What makes this report particularly interesting, however, is the rich political subtext that exists in the interstices of the report, popping up here and there in brief comments and barely suppressed yelps of exasperation. To understand this context, it is useful to reflect for a moment on the special nature of the IOM (...) and its relationship to government.IOM is part of the National Academy of Sciences, and is a private, non-governmental organization that does not receive direct federal funding for its work. Rather, IOM studies are often funded by contract with governmental entities that request reports on particular topics. As a case in point, the dietary supplement study was requested and paid for by the FDA. (shrink)

The United States has recently made significant and positive civil rights gains for LGB people, including expanded recognition of marriages between people of the same sex. Among the central tropes that have emerged in the struggle for the rights of LGB people are that they are “born that way,” that sexual orientations cannot change, and that one's sexual orientation is not affected by choice. Writer Andrew Sullivan put it this way: “[H]omosexuality is an essentially involuntary condition that can neither be (...) denied nor permanently repressed.… [S]o long as homosexual adults as citizens insist on the involuntary nature of their condition, it becomes politically impossible to deny or ignore the fact of homosexuality.… [The strategy for obtaining LGB rights is to] seek full public equality for those who, through no fault of their own, happen to be homosexual.”This idea of linking LGB rights to empirical claims about sexual orientations has become so central that casting doubt on these claims is, in many circles, tantamount to opposing LGB rights. Nonetheless, claims about innateness, immutability, and lack of choice about sexual orientation should not be the primary basis for LGB rights. (shrink)

In Thomas Thomson's System of chemistry of 1802 Bergman and Scheele are actually considered as creators of the analytical concept of an element. With regard to this, a detailed investigation of the work of Bergman and Scheele shows that Thomson's statement contains mistakes as well as inadmissable simplifications and generalizations. It is correct, however, that Bergman in 1774–1777 specifically anticipated in essential aspects the analytical element concept proposed by Lavoisier in 1787–1789.
